Most systems log events. Far fewer can reconstruct a decision. The difference is whether the inputs, the model version, the thresholds in force and the human actions taken are recoverable together as one coherent account.
That reconstruction is the whole point. If it takes an engineer a week and a database export to explain one output, the system is not auditable in any sense a regulator would accept.
